PHYSIOTHERAPY SYMBOLS

Physical therapy symbols are also being used in PT notes writing. They are often seen in physical therapy documentations as part of the patient medical chart.

Some of the most common PT symbols used in SOAP notes writing are the following:

+1, +2 - assitance

♂ - male

♀ - female

↓ - down, downward, decrease, diminished

↑ - up, upward, increase

// - parallel, parallel bars (also // bars)

s - without

a - before

~ - approximately

@ - at

∆ - change

> - greater than

< - less than

= - equal

+ - positive

-  - negative

# - number (e.g., #1) or pounds (e.g., 5# wt.)

% - percent
